HALT Diabetes is delivered online through the HALT 360 platform, using asynchronous sessions that participants can complete on their own schedule, plus support from a lifestyle coach via text, phone, or video.

The goal is to help participants lose at least 5–7% of their starting body weight and achieve at least 150 minutes of moderate physical activity per week, which has been shown to significantly reduce the risk of developing type 2 diabetes.

The program lasts 12 months. Participants meet weekly in the early months and then transition to less frequent sessions focused on maintenance and long-term support, consistent with CDC National DPP standards.

HALT Diabetes is designed for adults with prediabetes or at high risk for type 2 diabetes, such as those with elevated A1C, a history of gestational diabetes, or other risk factors identified by their healthcare provider.
