What Is Fruit Street – Diabetes for Medicare?

Fruit Streets Medicare Diabetes Prevention Program is an MDPP-approved, distance-learning version of the CDCs National DPP, designed specifically for Medicare beneficiaries with prediabetes.

Participants enroll in a year-long, evidence-based lifestyle change program that focuses on:

  • 26 live group classes over 12 months, led by a registered dietitian

    • Core phase in the first 6 months
    • Maintenance phase in the second 6 months
  • Small groups (up to ~30 participants) for accountability and peer support

  • Wireless scale provided at no cost to Medicare participants for remote weight tracking

  • Food logging and physical activity tracking via mobile app

  • Photo-based meal logging and activity device syncing

  • Secure text messaging with dietitians and group support between sessions

Who Is Eligible?

Eligibility is defined by CMS for the Medicare Diabetes Prevention Program. Medicare beneficiaries must:

  • Be enrolled in Medicare Part B

  • Have prediabetes based on qualifying lab values:

    • A1C 5.7–6.4%
    • Fasting plasma glucose 110–125 mg/dL
    • 2-hour plasma glucose 140–199 mg/dL after a 75g OGTT
  • Have a BMI ≥25 (≥23 if self-identified as Asian)

  • Have no prior diagnosis of type 1 or type 2 diabetes (except gestational diabetes)

  • Have no end-stage renal disease (ESRD)

Medicare covers MDPP once per lifetime for eligible beneficiaries.

Fruit Street offers simple, low-burden workflows for referral, enrollment, and reporting.

For clinicians and practices:

  • Patients can self-enroll online using their Medicare ID

  • Clinicians may also refer patients via online referral form or fax

  • The same year-long program is available out-of-pocket for $500

  • Equivalent to approximately $19 per session

  • Ideal for adults with prediabetes who do not yet qualify for Medicare
