HALT360 Diabetes is a digital, inclusive way to deliver the CDC’s National Diabetes Prevention Program (National DPP) lifestyle change program to adults with prediabetes or at high risk for type 2 diabetes. It combines evidence-based curriculum, live coaching, and an easy-to-use online platform to help participants build lasting healthy habits, wherever they are.
HALT 360 programs are also available in Native American and Spanish.

Organizations can start by requesting a demo and consultation to determine fit, implementation options, and licensing. From there, ProVention Health provides training, onboarding, and ongoing support for your coaches and administrative teams.
Many organizations use CDC-recognized National DPP programs to seek coverage or reimbursement from payers such as Medicare, Medicaid, and commercial health plans, depending on state and plan policies. HALT360 can support data collection and reporting needed for recognition and coverage discussions.
The program lasts one year, with more frequent sessions in the first six months and less frequent maintenance sessions in the second six months, following CDC National DPP standards. HALT360’s digital format allows participants to complete lessons on their own schedule between coach touchpoints.
HALT360 Diabetes serves adults 18 and older with prediabetes or at high risk for type 2 diabetes (based on blood tests, a history of gestational diabetes, or a high score on a prediabetes risk test). People with type 1 or type 2 diabetes are generally referred instead to Diabetes Self-Management Education and Support (DSMES) programs for ongoing management.

The National DPP is a CDC-led, evidence-based lifestyle change program that helps adults at high risk for type 2 diabetes make lasting changes in diet, physical activity, and stress management. Participation can significantly reduce the risk of developing type 2 diabetes, especially when participants lose 5–7% of body weight and increase physical activity to at least 150 minutes per week.
