Our course equips you with practical tools, engaging digital content, and ongoing support to help participants make small, sustainable lifestyle changes that prevent or delay type 2 diabetes. The National Diabetes Prevention Program (National DPP) lifestyle change program is a year-long, evidence-based program that helps adults with prediabetes or at high risk for type 2 diabetes eat healthier, move more, manage stress, and lose 5–7% of their body weight, which can lower their risk of developing type 2 diabetes by up to 58% (71% for people over age 60).

As a lifestyle coach, you will learn how to adapt your sessions, communication, and goal setting so participants with disabilities and other unique needs can fully engage and succeed. By delivering prevention through HALT 360’s online platform and app, you help remove common barriers like transportation, childcare, elder care, and work schedules while empowering people to understand the connection between daily choices and chronic diseases such as type 2 diabetes, hypertension, obesity, and arthritis.

Training is delivered virtually, so you can join from anywhere and quickly start leading HALT Diabetes cohorts in your organization or community using the HALT platform.

Potential participants can be referred by a health care provider or self-refer if they meet the eligibility criteria. Many programs also use tools like the CDC/ADA Prediabetes Risk Test and community screenings to identify people who could benefit from joining.
The National DPP lifestyle change program focuses on preventing or delaying type 2 diabetes in people with prediabetes or at high risk, before a diagnosis occurs. Diabetes Self-Management Education and Support (DSMES) is for people who already have diabetes and need ongoing education and support to manage their condition day to day.cdc+3
On average, people with prediabetes who complete the program and achieve modest weight loss through healthier eating and increased activity reduce their risk of developing type 2 diabetes by about half. Many participants also report more energy, better understanding of nutrition and physical activity, and improved overall health.cdc+1
Sessions are led by trained Lifestyle Coaches, who may be health professionals or other trained staff experienced in supporting behavior change. Coaches guide participants through the curriculum, foster a supportive group environment, and help each person apply strategies in their everyday life.
The National DPP lifestyle change program can be offered in person, online, through distance-learning (like virtual classes or telehealth), or in a combination of these formats. All delivery modes follow a CDC-approved curriculum and are led by trained Lifestyle Coaches who facilitate group discussion, provide support, and help participants set realistic goals.cdc+1
Participants learn practical ways to eat healthier, become more physically active, manage stress, problem-solve barriers, and stay motivated over time. The curriculum is based on the original Diabetes Prevention Program research trial and follow-up studies, which demonstrated that lifestyle changes can significantly delay or prevent type 2 diabetes.cdc+2
The Lifestyle Change program lasts one full year. During the first 6 months, participants usually attend at least 16 weekly sessions; in the second 6 months, they attend at least 6 sessions, often held monthly, to maintain progress and build long-term habits.cdc+1
The program is designed for adults 18 and older who have prediabetes or are at high risk for type 2 diabetes but do not yet have type 1 or type 2 diabetes. Participants typically have a body mass index (BMI) of 25 or higher (23 or higher for Asian Americans) and either qualifying blood test results, a history of gestational diabetes, or a positive score on the CDC/ADA Prediabetes Risk Test.
